St Clare reaches out for more Hospice at Home referrals
St Clare Hospice is celebrating the fifth anniversary of its Hospice at Home service.
More than 2,000 patients with life-limiting diseases have been given free, short-term care in the comfort of their own home since the Hastingwood-based charity launched the service in January 2014.
In this time, the team of registered nurses and nursing assistants also provided compassionate support for patients’ families and carers.
Hospice at Home manager, Sonia Haigh, is now calling upon local medical, social and voluntary professionals to make even more referrals to the Hospice at Home service. She said: “Hospice at Home care can help to avoid unnecessary hospital admissions, provide short-term care for rapid discharge from hospital while awaiting a care package, and provide hands-on compassionate care in the last weeks of a person’s life in the comfort of their own home.”
Referrals can be made by all health, social and voluntary care professionals, including GPs, district nurses, clinical nurse specialists, social care or hospital teams. Patients, their family, or carers can also refer themselves directly. The team will then need to discuss the referral with the patient’s GP.
